We are looking for just the right RN to support people to redesign their lives as they transition from institutional living into community and start to create a life with more independence and choice.

JOB SUMMARY:

CONNECT Partners will be supporting 44 individuals who are moving out of the George Pearson Centre (institutional care) into their own homes in the Cambie Gardens apartments (some single occupancy, most shared in 4- and 6-bedroom arrangements). We will support these individuals, all of whom have complex health needs with varying levels of independence and all of whom are in wheelchairs, as they move into community and start to build lives with more choice and autonomy.

· At the Cambie Gardens apartments we will be working between 12 apartment units within two neighbouring towers. 8 of those apartments are group homes with 4 bedrooms or 6 bedrooms.

· Each person we support will have their own bedroom and bathroom with ceiling lifts installed.

· There are support staff (Life Redesign Coaches) in each apartment 24/7. There is also a full professional coaching team, including OT, PT, SLP, Social Worker, Recreation Therapy.

· As a Health and Wellness Coach (LPN/RN) you will have a dynamic role. You will be moving between both buildings as needed to support with more complex clinical work that the support staff are unable to do. Your role will include hands on with residents, supporting the staff, and opportunities to engage the people we support and their families.

· We will provide additional tracheostomy and ventilator training.

The Health and Wellness Coach serves as a nurse, coach, mentor and teacher for the people we support, families, and co-workers, informs the development of appropriate support strategies for individuals and staff, providing nursing skills as needed for the individual’s health care needs and lifestyle choices.

The Health and Wellness Coach contributes to CONNECT Partners’ orientation and continuous learning program, and helps build on standards for excellence, and commitment to continuous quality improvement.

REPORTS TO: Leader – CONNECT Partners

JOB REQUIREMENTS & COMPETENCIES:

· A Nurse registered and in good standing with the BC College of Nurses and Midwives able to support individuals who require responsive agile nursing support;

· Support residents with stable tracheostomies and ventilators who are able to live in community (an RN will provide in house theory and hands-on training);

· Support residents with stable gastrostomy tubes, tube feeds, suprapubic catheters, and ostomies (an RN will provide in house theory and hands-on training);

· Authentic, intuitive, and able to build sustainable, productive relationships with the people we support, their families, and all members of the individual’s service resource team as well as other professionals involved in their care;

· Flexible, adaptable, creative, resourceful and resilient, able to competently juggle and manage multiple, sometimes competing priorities while maintaining their sense of perspective and fun;

· Must be comfortable working together with Life Redesign Coaches giving them the support they need and be available to them out of hours as part of an emergency/urgent support leadership network.

· Willing to walk beside individuals coaching and assisting them to set goals and create a life that is meaningful to them.

· Must be fully vaccinated against Covid-19

KEY TASKS & RESPONSIBILITIES

· Understand a person’s story and the factors that influence their priorities, choices and life goals; bring forward your thoughts, ideas and recommendations for consideration and inclusion as measurable, actionable steps to success in their individual life redesign plan;

· Provide support and mentorship to co-workers and staff in relation to clinical skills and supporting residents;

· Ensure that what is brought forward, whether it is to be implemented by the Health and Wellness Coach or by someone else, has been designed collaboratively, and in partnership with the individual to help them achieve their overarching goals and to maximize their participation in real life, relevant and meaningful activities, equitable relationships and community;

· Make best use of emergent opportunities for the individual’s real-life learning;

· Show others how they can support the Life Redesign process;

· Collaborate with colleagues so the collective efforts equate to one integrated plan that will help create positive new content for the next chapter of the person’s story;

· Manage actions and time to successfully meet the demands and expectations of the role;

· Provide the Leader with regular updates on work priorities along with any recommendations for quality improvement.
