Summary of DutiesThe Registered Nurse will coordinate, and support patients referred to the Sunnybrook LDAP during their early cancer journey. This is a very important role, with the opportunity to make a significant positive impact on patient care.Responsibilities include triaging referrals, completing telephone assessments, identifying potential complications and booking necessary diagnostic/staging tests. The RN will be responsible for following up on these tests and making sure patients have follow-up appointments and referrals booked. The RN will work closely with physicians to monitor the progress and wait times for each patient. The RN may participate in multidisciplinary case conference rounds as well as attend any courses or conferences applicable.The RN will have face-to-face contact with patients in clinic, assisting with patient intake, medication assessments, vital signs, supporting MDs with procedures, and providing counseling/education around the lung cancer diagnostic process and various diagnostic procedures.The RN will be the main contact for LDAP patients, answering questions about test bookings and communicating new concerns or cancer symptoms to the LDAP physician so these can be managed in a timely manner, avoiding ED visits.The RN will work together with administrative staff to maintain an LDAP patient database and track quality metrics, book tests, arrange referrals, bronchoscopy bookings and ensure the clinic has adequate procedural supplies.Qualifications/Skills
- Current registration with the College of Nurses of Ontario
- Baccalaureate Degree in nursing preferred
- Experience in nursing required; minimum of five (5) years related practical experience
- Experience in ambulatory care, critical care, emergency nursing preferred
- Patient Navigation Certification or willing to obtain within a year of hire preferred
- Experience assisting MDs with procedures preferred
- Capacity to act as a member of the interprofessional team
- Ability to work independently
- Excellent interpersonal, oral and written communications skills
- Experience with virtual care and project management considered an asset
- Proficient in Microsoft products (Excel, Outlook, PowerPoint)
- Proficiency in electronic medical record use preferred
- Experience in following medical directives preferred
