Your Opportunity:

The Pharmacist I is responsible for the provision of safe, effective, and efficient preparation and distribution of pharmaceutical services to patients in the ambulatory care setting – Home Parenteral Therapy (HPT) Program. The Pharmacist I manages medication therapy for patients in which he/she, both initially and on an ongoing basis, assesses patients for potential or actual drug-related problems, working within a collaborative team environment, and intervenes to resolve or prevent drug-related problems in order to optimize patient outcomes. This is accomplished through actively participating in daily program huddles, liaising with members of the health care team, and developing a professional relationship with the patient and their family to formulate a medication treatment plan. This also entails implementing a plan, including selecting, initiating, modifying, prescribing, adapting, or administering medication therapy, documenting expected and real outcomes, and documenting clinical interventions. The Pharmacist I is responsible for the provision of drug information and medication counselling services, ensuring accurate and efficient dispensing of medications, and performing administrative responsibilities. This position will provide support to the HPT program based out of the Royal Alexandra Hospital, University of Alberta Hospital and Kaye Edmonton Clinic. The incumbent acts as a preceptor and mentor for pharmacist students and residents completing their rotations, as well as other Pharmacy staff.

Required Qualifications:

Bachelor of Science in Pharmacy. Active or eligible for registration with the Alberta College of Pharmacy (ACP).

Additional Required Qualifications:

Additional Prescribing Authorization is required. Completion of an accredited hospital residency program or Pharm D program or at least one-year recent experience in a large acute care hospital pharmacy practice is required. Recent experience in a proactive clinical practice as a part of a multidisciplinary health care team required. Fluency in the English language (both written and verbal) required. Computer literacy in drug information and word processing required. Demonstrated ability to use evidence-based medicine in direct patient care is required. Ongoing participation in self-directed learning and purposeful professional development is required. Excellent communication and interpersonal skills, with demonstrated organizational and problem-solving skills are required. Capable of making decisions and working independently. Good time management skills, including ability to prioritize, switch priorities as needed, and multitask are required. Self-motivated; able to work both independently and cooperatively in a team environment.

Preferred Qualifications:

Active membership in the Canadian Society of Hospital Pharmacists (CSHP) recommended. Current competency in sterile product preparation is preferred. Competence with the Connect Care and Kroll computer system is preferred. HPT experience preferred.
