Job description

The Streets to Home Program provides housing for vulnerable people who have been street involved, experiencing mental health and/or addictions issues. The program provides onsite psychosocial, primary care support and services including intensive case management; harm reduction and addiction supports; therapeutic and supportive counseling; 24-hour staffing; assistance with activities of daily living; nursing services; life skills development; social and recreational activities; volunteer opportunities; and follow up support etc.What You Will DoHarm Reduction Education and Support:

  • Deliver harm reduction education tailored to residents’ needs, focusing on safe substance use practices, overdose prevention, and community resources.
  • Promote a non-judgmental, compassionate approach to support residents with substance use challenges.
  • Empower residents by emphasizing their autonomy in making informed choices about their recovery and treatment options.

Client Support and Advocacy:

  • Provide one-on-one support to residents, assisting them in setting and achieving harm reduction goals.
  • Build trusting relationships with residents, offering both emotional support and practical assistance.
  • Advocate for residents by connecting them to critical external services, including addiction counseling, medical care, mental health resources, and social services

Resource Coordination:

  • Ensure timely access to harm reduction supplies, such as safe injection equipment, naloxone kits, and personal protective equipment (PPE).
  • Manage and maintain an up-to-date inventory of all harm reduction supplies; ensure timely replenishment as necessary.
  • Educate residents on proper disposal of needles and safe handling techniques to ensure a safe environment.

Crisis Intervention:

  • Respond promptly to health and safety crises, including overdose management and prevention.
  • Collaborate with other staff and external professionals during emergency situations to provide immediate support​​​

Community Building and Support:

  • Facilitate group discussions, workshops, and community-building activities that emphasize harm reduction and wellness.
  • Foster peer-to-peer support networks among residents, building a sense of community and mutual respect.

Documentation and Reporting:

  • Manage harm reduction folders and accounts with internal/external partners to ensure that all necessary documentation, records, and service usage are kept accurate and up-to-date.
  • Maintain confidential and accurate records of interactions with residents, documenting assessments, progress notes, and follow-up actions.
  • Prepare detailed reports for program evaluation, ensuring adherence to policies and regulations.

Collaboration and Training:

  • Work closely with case managers, internal support staff, and external community service providers to ensure a holistic approach to client care.
  • Participate in ongoing training and professional development related to harm reduction, substance use, and community health.

What You Bring to the Team

  • Education: Bachelors in Social Work, Sociology, Psychology or a related discipline.
  • Experience: Minimum of 3 years of experience working in harm reduction, addiction services, or a similar field.
  • Expertise: In-depth knowledge and experience in harm reduction practices, specifically in overdose prevention, safe substance use, and crisis intervention. Also, knowledge of the following: stimulants, opiates & injection drug use issues, alcohol use and drug use
  • Demonstrated expertise in harm reduction, trauma-informed care, and anti-oppression practices.

What Will Set You Apart

  • First Aid/CPR and Naloxone administration training.
  • Proficiency in crisis intervention, conflict resolution, and advocacy, particularly with marginalized populations such as individuals experiencing homelessness, mental health challenges, or substance use.
  • Strong ability to mentor and support peers, volunteers, and clients, promoting a collaborative and supportive work environment.
  • Excellent communication skills (written and verbal), with the ability to work with diverse populations and develop educational resources.
  • Familiarity with local resources related to substance use, mental health, and social services.
  • Additional Assets:
  • Lived experience with substance use, homelessness, or street involvement is valued.
  • First Aid/CPR and Naloxone administration training.
