I)Day Treatment CentrePosition Type: Temporary Part-timeContract: 6 monthsShift Type: Days (Shifts may be subject to change)Hourly Pay Rate: $44.22 – $57.35Bi-Weekly Hours: 28 HoursHours of Work: 7 hrs/shiftPosting Number: 8972Union: OPSEU595Date Posted: May 30, 2025Internal Closing Date: June 6, 2025Job summary:Ambulatory Services assists older adults who are having difficulty managing in their own homes and have complex health problems requiring rehabilitation. The goal of the treatment is to help clients attain and maintain optimal physical and psychosocial functioning.Responsibilities include but are not limited to:

  • Provides individual, family and/or group psychosocial assessments, support and counselling for both programs
  • In collaboration with the inter-professional team, implements a client focused care plan and coordinates the discharge planning process for the patients
  • Provides support/consultation to other disciplines on the team re: psychosocial issues particularly related to complex patient/family situations
  • Crisis intervention
  • Acts as a linkage and communication conduit between the family and the care team
  • Establishes community linkages and outreach with relevant agencies and other Baycrest programs and services
  • Participates in teaching and other student supervision activities for health professional students assigned to DTC/ICCT
  • Provides support and expert knowledge to other outreach teams
  • Collaborates with GPCS and IPCT to identify and provide coordinated care to patients who require multiple outreach services

Qualifications include but are not limited to:

  • Masters of Social Work degree required
  • Current and valid registration with the Ontario College of Social Workers and Social Service Workers (‘OCSWSSW’) in good standing required
  • Access to transportation (e.g. current driver’s license and car) required
  • Experience with identifying and assessing the use of clinical assessment tools for clinical and research objectives related to caregivers
  • Experience working with caregivers across clinical settings and the spectrum of supports (i.e. prevention, crisis, self-management, treatment etc.) and a comprehensive understanding of the diverse needs of caregivers
  • Active understanding of community partners and integration opportunities related to caregiving
  • Excellent interpersonal, communication and problem solving skills
  • Computer skills including Outlook, Meditech, Point Click Care, Word, Excel, Resource Matching and referral (RM&R), and MDS preferred
  • Knowledge of Zoom and OTN video conferencing technologies are a very strong asset.
  • In-depth experience in research activities as they relate to innovative forms of social work intervention with caregivers.
