Baycrest Hospital has an opportunity for aREGISTERED NURSE (RN)Geriatric Psychiatry Community Service (GPCS)Position Type: Permanent Part-TimeShift Type: Days, no weekends (Shifts may be subject to change)Hourly Pay Rate: $39.07 – $56.00Bi-Weekly Hours: 37.5 hoursHours of Work: 7.5 hrs/shiftPosting Number: 8365Union: ONADate Posted: November 29, 2024Internal Closing Date: December 6, 2024Job Summary:The Registered Nurse will be responsible for health teaching and health promotion and will act as one of the principle case managers for clients within the GPCS.Responsibilities include but are not limited to:

  • Provides sound clinical knowledge and assessment skills, acts as clinical expert and resource and provides comprehensive, quality client/family centred health care services from admission, through transfers and discharge
  • Acts as a case manager to clients and their families
  • Connects clients and their caregivers/families to community resources that will help the client to live well in the community and manage their illness
  • Plans for, delivers and evaluates nursing interventions, including medications, psychoeducation and intervention, as well as treatments which fall within the RN’s scope of practice
  • Participates actively in the teaching programs for students including, but not limited to nurses and nursing students
  • Actively participates in program development and evaluation
  • Able to drive to visit client’s and families in their homes within the GPCS catchment area

Qualifications include but are not limited to:

  • Current registration, in good standing, from the College of Nurses of Ontario (‘CNO’) is required
  • Current Basic Cardiac Life Support (‘BCLS’) certificate, Level C – Automated External Device (‘AED’) is required
  • BScN preferred
  • Demonstrated ability to take professional accountability for leading, organizing, providing and delegating care activities for clients and families
  • Psychogeriatric experience, i.e. mental health assessment and intervention and working with BPSD preferred
  • Knowledge of community resources for clients and families
  • Demonstrated ability to work independently and within an interdisciplinary team
  • Excellent interpersonal, communication and problem-solving skills
  • Basic computer skills – Meditech, Microsoft Word, Microsoft 365, and Excel preferred
