Your Opportunity:

This Pharmacist I position offers an opportunity to work in collaboration with infectious disease physicians, nurse practitioners, nurses, social workers, dieticians, and other clinical pharmacists to provide care for patients who are HIV positive at the Northern Alberta Program (NAP) – Kaye Edmonton Clinic. NAP offers assessment, treatment, education and, support for people who have HIV in an ambulatory clinic environment, including providing access to anti-retroviral medication. The pharmacist working to full scope of practice, reviews medication therapy for patients by assessing for potential or actual drug-related problems to optimize medication therapy and patient outcomes. This is accomplished through actively liaising with members of the health care team and developing a professional relationship with the patient to formulate a patient-centered medication treatment plan. This also includes selecting, initiating, modifying/adapting medication therapy, prescribing; documenting expected and real outcomes via monitoring plans; and documenting clinical interventions and performing seamless care activities including liaising with the patient’s community pharmacy and the Rexall Outpatient Pharmacies. The pharmacist will also be involved in rapid antiretroviral medication starts, engaging with patients who have been newly diagnosed, those who are re-engaging in their care and new immigrants to Canada. The pharmacist must be able to triage and prioritize patient care along with other competing demands. The pharmacist is responsible for the provision of drug information, medication counselling services and performing administrative responsibilities which may include completion of Specialized High-Cost Drug Funding Program enrollment forms.

As a Pharmacist I, you will be responsible for the safe, effective and efficient preparation and distribution of pharmaceutical services. You will manage medication therapy for patients, both initially and on an ongoing basis. In addition, you will provide assessment, intervention and resolution of potential or actual drug-related problems and oversee activities related to compounding and packaging.

Required Qualifications:

Bachelor of Science in Pharmacy. Active or eligible for registration with the Alberta College of Pharmacy (ACP).

Additional Required Qualifications:

Additional Prescribing Authorization. Fluency in the English language (both written and verbal). Computer literacy in drug information and word processing. Demonstrated ability to use evidence-based medicine in direct patient care. Ongoing participation in self-directed learning and purposeful professional development. Excellent communication and interpersonal skills, with demonstrated organizational and problem-solving skills. Capable of making decisions and working independently. Good time management skills, including ability to prioritize, switch priorities as needed, and multitask. Self-motivated; able to work both independently and cooperatively in a team environment.

Preferred Qualifications:

Recent experience in a proactive clinical practice as a part of a multidisciplinary health care team. Previous infectious disease or HIV specialty experience. Previous experience with Connect Care. Completion of an accredited hospital residency program or Pharm D program.
